2010: Evaluated a Chronic Disease Prevention Program for a rural county in Massachusetts.
2011: Implemented ED Diversion programs to reduce healthcare costs pediatric (Medicaid) in SE Washington, DC.
2012: Served as a Care Coordination SME for the development of a Population Health Analytics tool.
2013: Developed Diabetes Self-Management programs for Medicaid recipients in Florida.
2014: Led a Care Transitions program on behalf of a primary care group to reduce Medicare Part A costs.
2015: Chartered a comprehensive Community Health Worker training program for a state primary care association.
2016: Implemented our first Population Health Tool migration and training for an Accountable Care Organization in California.
2017: Initiated our Internships and Clinical Rotations program with 5 participating universities.
2018: Designed a Care Management and Population Health Manual for a multi-practice organization.
2019: Provided Training and Technical Assistance for physicians, ACOs, and MCOs for the CMS - Transforming Clinical Practices Initiative (TCPI).
2020: Guided the implementation of virtual patient visits and telework policies in response to the COVID-19 pandemic.
2021: Became a training vendor for CDC University.
2022: Assisted an ACOs in significantly reducing Medicare costs and achieving a carve-out for care management.
2023: Developed our largest Medication Formulary and Medication Reconciliation strategy to lower Medicare Part D costs.
2024: Achieved PCMH Recognition for a 23-practice organization.
2025: Completed a Medical Economics playbook for an ambulatory care practice in Texas.
