Course Description:
Participants in this course will learn how to use health, claims, and demographic data to develop interventions for population health. The strategies taught in this training may be used to improve continuum of care and patient satisfaction. Mastery of this training may assist with Patient Center Medical Home, NCQA, or HEDIS compliance.
Learning Objectives:
  1. Identify the most appropriate data sources to be used for strategizing population health.
  2. Understand the ethnic, social, and cultural groups that require the most assistance.
  3. Determine the health conditions and services that are best addressed through population health.
  4. Develop strategies that align with the population served at your organization.
  5. Prioritize interventions and patients based on need and fiscal guidelines.
  6. Utilize data sources to evaluate and adjust population health strategies.
Continuing Education Units:
3 CECH credits with be offered to Certified Health Education Specialists
3 Generic CEUs will be provided to all other participants
